Most insurance plans cover our services.  

Due to our unique services, authorization may be granted regardless if we are listed as a provider in your plan.  

Please allow us to verify your insurance coverage prior to your first visit.  

Self-pay option is available.

Referral by physician is recommended but not required unless needed for insurance coverage.  

CANCELLATIONS We dedicate one-on-one time with your therapist and therefore require 48 hour notice of cancellation.  Failure to do so will result in a $40 charge not payable through insurance. 

To save yourself time, you may print, complete and bring it with you to your first visit.
